Community Outreach

Key Point Health Services is pleased to announce the development of a Community Outreach Program, designed to provide essential health services to persons with mental health concerns.

One of the most important aspects of this program is that it will provide coordination of services. This will greatly aid clients and care providers by decreasing missed appointments, monitoring use of medications and maintaining eligibility for entitlements such as medicare and medicaid.

Our outreach workers are experienced and skilled direct care providers who are especially trained to assess the needs of clients, and, with the client's input, develop a customized plan to meet the individual's needs.

This personal relationship between the outreach worker and the client goes a long way to not only reduce anxiety on the part of the client, but also reduces unnecessary burden on the medical care provider by being available to answer questions and allay concerns.

Our Community Outreach Program supports clients in their own environment by providing the following services and support:

  • Life skills training
  • Independence workshops
  • Transportation
  • Social events
  • Assistance with scheduling and maintaining appointments
  • Medication monitoring
  • Liaison and interpretation of other services and care providers
With the addition of this exciting new outreach program, our mission is extended to allow participants to function in an independent environment with the least intrusive care possible.
