How the Adult Psychiatric Rehabilitation Program (PRP) and Community Outreach Program Work Together to Support Recovery

Both the Adult Psychiatric Rehabilitation Program (PRP) and the Community Outreach Program at Keypoint Health Services serve adults aged 18 and older who are living with serious and persistent mental illness. To qualify for either program, individuals must have Maryland Medicaid, be actively engaged in outpatient therapy, receive a referral from a licensed behavioral health professional, demonstrate a willingness to participate, and have a diagnosis that meets the criteria for the priority population.

Each program shares a core focus on recovery, promoting independence, supporting community integration, developing practical life skills, and preventing hospitalization or behavioral health crises. Common services offered across both programs include rehabilitation assessments, case management, individualized treatment planning, group counseling and structured activities, social skills development, health education, computer literacy training, recreational opportunities, and transportation assistance.

Community Outreach Program

Flexible, in-home psychiatric rehabilitation services that support adult independence, housing stability, and access to essential community resources.

This community-based program offers flexible, in-home and mobile services to adults with chronic mental illness.

Outreach staff help clients navigate everyday life, access community resources, and develop greater independence.

  • Substance use support (SUD), homelessness prevention
  • Transportation to/from community locations (meals not included)

Eligibility & Similarities

  • Adults (18+) with serious and persistent mental illness
  • Maryland Medicaid required
  • Active participation in outpatient therapy
  • Referral from a licensed behavioral health professional
  • Must have a priority population diagnosis
